Service Description
Non-evidential breath alcohol screen using a professional-grade fuel-cell breathalyzer. Results provided in minutes with a signed results form. Available in-clinic or mobile. Not for DOT or law-enforcement use. What’s included • One breath alcohol screen (up to 2 attempts if needed) • 15-minute observation if a confirmation screen is required by your policy • Signed results form with the test date/time, device ID, and BAC reading • Photo ID verification and chain-of-custody style documentation upon request • Option to add a LabCorp alcohol metabolite (EtG/EtS) urine test for confirmation or longer detection window (additional fee)
Cancellation Policy
At Prime Diagnostics, we value your time and strive to provide reliable, professional service for every client. Please review our policies below: 📌 Booking Appointments • All services must be booked in advance through our website or by phone. • Same-day appointments may be available depending on service type and staff availability. 📌 Cancellations & Rescheduling • Clients may cancel or reschedule up to 24 hours before their scheduled appointment at no charge. • Cancellations made less than 24 hours in advance may be subject to a cancellation fee. • No-shows (missed appointments without notice) are non-refundable. 📌 Payments • Payment is due at the time of service. • For online bookings, we may require a card on file to secure your appointment. • Any cancellation or no-show fees will be charged to the card on file. 📌 Arrival • Please arrive 10–15 minutes early for your appointment to allow time for check-in and paperwork. • Clients arriving more than 15 minutes late may need to reschedule. 📌 Additional Notes • All services are performed in a professional, confidential, and secure environment. • A valid photo ID may be required for certain services (such as drug testing and background checks).
